Medical Records
Our Health Information Management department processes, stores and retrieves patient medical records. Current patients may also access their health records through MyChart. Sign up for MyChart here.
To request a copy of your medical record or your child’s record for personal use, please choose an option below. Be sure to include:
- patient’s full name
- date of birth
- specific records requested
- treatment dates
- your name, address, phone number and relationship to the patient
Print the blank form, complete by hand and sign.

Complete the fillable (pdf) form using the required information above and sign using an e-signature (must include a photocopy of a government issued ID for proof), or print and hand sign.

Sending records to Akron Children's?
Use one of the forms below.
The policy of Akron Children's is to maintain the first-registered patient name in the medical record until we receive legal documentation that allows a name change. Due to the timing of some patient registrations, we may have a name on file that has never been the legal name of you or your child. To ensure we have one patient per medical record, we require legal documentation to prove a name change in our system or correct any name discrepancies in the patient’s record.
If the patient's name in the medical record or MyChart does not match their current legal name, you may provide any of these documents to request a name change: birth certificate, court order, divorce decree, driver's license, finalized decree of adoption, marriage certificate, Medicaid insurance card, passport or social security card.

Additional Medical Record Changes
If you need to request a change to a medical record other than a name change, please download, complete, sign and send the amendment form to records@akronchildrens.org
The vital records office in the county you/your child was born keeps all vital records like birth certificates. Most of them have an online request form where you can complete and pay for a request for a copy of a birth certificate.
You are just a few steps away from secure, 24/7 access to your family’s health information.
The next step in the process is to access the on-line consent/enrollment form located at: https://mychart.akronchildrens.org/
Click on the purple button: “Request Access Online”
To sign up for an account, please complete and submit the appropriate enrollment form based on patient age and relationship. Once your request has been received and processed, you will receive an activation email.
Our Medical Records Department will contact you by phone or email if they have any questions.
Our MyChart Call Center Support Team is available at 330-543-4400 for any questions you have while completing the on-line form. Their office hours are 7:00am - 5:00pm Monday through Friday.
If you prefer, you can also sign-up and complete an enrollment form at any of our Akron Children’s Pediatric Offices.

You can either contact your pediatrician for your child’s vaccination records, log in to your MyChart account and click on “Immunizations” for the vaccine records or obtain them through the medical records department.
