In order to serve you and your family to the best of our ability, we would like to have your child’s medical records sent to us. The link above contains a partially pre-filled HIPAA authorization form. Please fill in the blank sections that apply, sign and date the form – and give it to your child’s current pediatrician’s office. They will then send over the information we need. If you have any questions about the form, please contact our Health Information Management department at 330-543-8552.
