Phones turn off 30 minutes before the office closes.

Flu and COVID-19 vaccines are available for kids 6 months and older. MyChart users can click here to schedule a vaccine appointment. If you don't have MyChart, click here to schedule online or call 330-543-2778.

To keep our families and staff safe, please review these guidelines before arriving for an appointment.
- Visitor restrictions: The patient and up to 2 healthy adults are permitted.
- Arrive wearing a mask: Everyone but kids under age 2 and those with sensory issues should wear masks or face coverings. Cloth masks are permitted. We will provide a mask if you don't have one.
