A scheduler from the Appointment Center will call your patient within 1-2 business days after receipt of your referral order to schedule an appointment.
Ambulatory Referrals
If you are a physician or staff requesting an appointment for your patient, use the form below. A member of our scheduling team will call your patient within 1-2 business days to schedule an appointment. This form is not required to refer a patient. It also is not intended to replace Epic ambulatory referral orders.
This form is not for urgent health problems, same-day appointment, or appointment cancellations.

Ordering a Radiology Procedure
Complete the Radiology Order Sheet and fax to 330-543-3250 prior to scheduling for all locations. When possible, include the prior authorization number to ensure prompt scheduling.

Joint Hypermobility Genetics Referral
Joint hypermobility (JH), in isolation, is often a common trait that does not indicate the need for genetic testing. These patients may still experience symptoms from JH. Patients may benefit from a referral to a physician who is knowledgeable regarding the symptoms and treatment for JH.

Eligible families can also access community resources onsite at our Family Resource Center. Departments onsite include: Summit County Department of Job and Family Services, Summit County WIC Clinic and United Way of Summit & Medina. View services provided here.
Providers can fax prescription forms, anthropometric information or medically fragile patient letters to the WIC Clinic at Akron Children's Hospital at 330-752-7812.
