Objectives (Educational Content) :

1. Summarize the hospice benefit model. 2. Identify benefits and challenges in pediatric hospice care. 3. Examine the process of care team handoff to hospice team.

Target Audience:

General pediatricians, family physicians, nurse practitioners, physician assistants, social workers, psychologists, and nurses.

Identified Gap:

Many clinicians lack sufficient knowledge and skills to effectively navigate the hospice benefit model, address the unique challenges of pediatric hospice care, and coordinate seamless transitions to hospice teams, resulting in fragmented end-of-life care for patients and families.

The learner will view the presentation, successfully complete a post-test and complete an activity evaluation.
