Laboratory monitoring of LMWH is usually not necessary. In certain clinical situations (i.e. obesity or renal failure) monitoring may be indicated. Chromogenic antiXa assay is the recommended test. This assay should be obtained 4 hours after the dose is administered. Assays within 1 to 2 hours of dose are often very low as are tests > 8 hours after dose. (there is no "steady state") A conservative therapeutic range is 0.61.2 IU/mL.
