Laboratory monitoring of LMWH is usually not necessary. In certain clinical situations (i.e. obesity or renal failure) monitoring may be indicated. Chromogenic anti-Xa assay is the recommended test. This assay should be obtained 4 hours after the dose is administered. Assays within 1 to 2 hours of dose are often very low as are tests > 8 hours after dose. (there is no "steady state") A conservative therapeutic range is 0.6-1.2 IU/mL.
