Grand Rounds: Demystifying Transition to Adulthood: Moving from Anxiety to Action

PRESENTATION BEGINS AT ABOUT THE 14 MINUTE MARK. CLICK AHEAD TO VIEW 1. Identify transitional needs for young adults moving out of pediatric care. 2. Explain the design used to meet this need. 3. Describe the clinical pathways to consider for optimal outcomes of patients’ transitions.

Target Audience:

General pediatricians, family physicians, nurse practitioners, physician assistants, social workers, psychologists and nurses.

Identified Gap:

Transition from adolescence to adulthood can be a time of great stress and uncertainty, especially for youth struggling with chronic healthcare issues.
